ما هو الألم العضلي الليفي
- الألم العضلي الليفي هو عبارة عن حالة طبية مزمنة أي متلازمة مزمنة، تدعى الـ”فيبروميالغيا” Fibromyalgia وتظهر بصورة آلام في العديد من المناطق العضلية في الجسم، في الأربطة وفي الأوتار
بالإضافة إلى التعب الشديد والألم الغير مبرر وقت حدوث أي تقلبات مزاجية للمريض.
- ويظهر أحيانًا كنتيجة لأزمة عاطفية أو إصابة جسدية ولكن في القسم الأكبر من الحالات ليس هنالك عامل مسبب لظهور الألم العضلي الليفي.
- الألم الليفي العضلي هو اضطراب يسبب ألمًا واسع المدى في البنية العضلية الهيكلية يصاحبه الشعور بالإرهاق واضطرابات النوم والذاكرة والحالة المزاجية، لكن يعتقد الباحثون أن الألم الليفي العضلي يزيد الإحساس بالألم عن طريق التأثير على طريقة معالجة الدماغ والحبل النخاعي للإشارات المؤلمة وغير المؤلمة.

- تبدأ الأعراض غالبًا بعد وقوع حادث كالإصابة الجسدية أو الجراحة أو العدوى أو الإجهاد النفسي الشديد. وفي حالات أخرى، تتزايد الأعراض تدريجيًا بمرور الوقت دون أي سبب محفز.
- النساء هن الفئة الأكثر عرضة للإصابة بالألم الليفي العضلي من الرجال، كما يعاني العديد من الأشخاص المصابين بالألم الليفي العضلي من الصُّداع التوتُّري أي الصداع الناتج عن التوتر والاضطربات النفسية، واضطرابات المفصل الصدغي الفكّي، ومتلازمة القولون المتهيج، والقلق، والاكتئاب.
- رغم عدم وجود علاج للألم الليفي العضلي، توجد مجموعة متنوعة من الأدوية تساعد في السيطرة على أعراضه، ومن الأمور المفيدة أيضًا ممارسة الرياضة وتمارين الاسترخاء وتدابير تقليل التوتر كما يمكن علاجه بالأعشاب أيضاً.
أسباب الألم العضلي الليفي
تختلف أسباب الألم العضلي الليفي من مريض لآخر وهي كالتالي:
أسباب وراثية
بما أنّه يبدو أن الفيبروماليجا تنتقل في العائلة من جيل إلى جيل، فمن المحتمل أن تكون هنالك علاقة على أساس خلل جينيّ وراثيّ قد يجعل الإنسان أكثر عرضة للإصابة بمرض الألم العضلي الليفي وبالأعراض التي ترافقه.
كدمة على خلفية نفسية أو جسدية
- ثبت وجود علاقة بين الألم العضلي الليفي وبين اضطراب الكرب التالي للرضح (Post traumatic stress disorder)، من المعتقد هذه الفترة بأن أساس المتلازمة يكمن في عامل الألم المركزي.
- وتقول هذه النظرية أن الناس الذين يعانون من حالة الألم العضلي الليفي تكون عتبة الإحساس بالألم لديهم منخفضة أكثر من غيرهم وذلك نظرًا لحساسية الدماغ لاستقبال إشارات الألم.
- أي أن المستقبلات الدماغية تعاني من ذاكرة قصيرة الأمد فيما يتعلق بالألم فتصبح نتيجة لذلك ذات حساسية مرتفعة مما يعني أنها تصبح على درجة مرتفعة من ردة الفعل المستمرة تجاه علامات ظهور الألم.
- يعتقد العلماء أن الإثارة العصبية المتكررة لدماغ الشخص المصاب تسبب تغيرات كيميائية معينة، وتظهر هذه التغيرات بصورة إرتفاع حاد واستثنائي في إفراز مركبات كيميائية معينة في الدماغ وظيفتها التنبيه من الألم.
أسباب نفسية
وقد تكون سبب الإصابة بمرض الفيبروميالجا هو سبب نفسي نتيجة لصدمة نفسية لسبب ما أو إصابة جسدية.
أعراض الألم العضلي الليفي
تسبب الفيبروميالجا العديد من الأعراض لمصابيها بسبب الحساسية المفرطة للألم، ومن أبرزها:
- التعب، والإرهاق العام، والاكتئاب، والقلق والصداع، ومشاكل النوم.
- ألم واسع الإنتشار في الأرباع الأربعة، أعلى وأسفل الخصر وجانبي الجسم، يستمر لمدة 3 أشهر على الأقل.
- ألم عند الضغط على ما لا يقل عن 11 من أصل 18 نقطة حساسة للألم محددة في جسم المصاب بـ”فيبروميالغيا”.
- إنتشار مناطق الوجع أو تصلب 3 نقاط تشريحية على الأقل لمدة 3 أشهر.
علاج الألم العضلي الليفي بالأعشاب
قد تفيد بعض الأعشاب ذات الأثر العلاجي في التخفيف من الأعراض المرافقة للالتهاب العضلي الليفي، مع التأكيد على وجود دراسات كافية لتأكيد تأثيرها، من هذه الأعشاب:

العرن المثقوب
- اشتهرت هذه النبتة منذ القدم بخصائصها المضادة للاكتئاب مما ساهم في استخدامها للتخفيف من الاكتئاب المرافق للإصابة بالفييبروماليجا
- يرجح استعماله بعد استشارة الطبيب بجرعة تتراوح ما بين 600-900 مليغرام يوميًا لمدة لا تتجاوز 12 أسبوعًا، وبالرغم من فوائده إلّا أنّها قد تُسبب آثارًا جانبية مختلفة، ومنها إسهال، دوخة، مشاكل في النوم
حشيشة الهر
- يعاني أصحاب هذه المتلازمة من أرق واضطرابات في النوم لذلك يلجأون إلى منقوع عشبة حشيش الهر (Valerian)، فقد بينت الدراسات أن هذه العشبة تقلل الوقت المستغرق للنوم وتعزز من جودة النوم
- تستخدم نبتة حشيشة الهر بجرعة 450-1410 ميلغرام يوميًا ولمدة تتراوح ما بين 4-8 أسابيع للحصول على هذه الفوائد، ولكن يجب الحرص على استخدامها بالجرعة الموصى بها لوجود أعراض جانبية منها صداع، ضعف وتعب عام، دوخة، إسهال، ألم في المعدة.
الزنجبيل
إن الالتهاب العضلي الليفي هو عبارة عن فرط حساسية للألم، فقد يساعد تناول الزنجبيل كشاي في التخفيف من الألم، وذلك لخصائصه المهدئة والمسكنة للألم

- يجب استشارة الطبيب قبل استخدام الزنجبيل إذ لا ينصح باستخدامه من قِبل الأفراد الذين يعانون من اضطرابات النزيف أو يستخدمون أدوية مميعة للدم، أو الأفراد الذين يعانون من مشاكل صحية مختلفة كالسكري، وأمراض القلب
- كما قد يسبب استخدامه بكميات مفرطة المعاناة من بعض الآثار الجانبية مثل حرقه في المعدة وشعور بعد الراحة، إسهال.
الكركم
- يحتوي الكركم على مادة الكركمين الفعالة ذات التأثير المسكن للألم، مما يساهم في تخفيف الألم المرافق لحالة الالتهاب العضلي الليفي
- تناول الكركم يعد آمنًا بوجه عام، إلا أنه غير آمن للحامل والمرضع وكذلك للمصابين ببعض الأمراض كالسكري، أمراض المرارة واضطرابات المعدة، حيث يتوجب عليهم طلب الاستشارة الطبية قبل تناوله

- قد تشكل عاملًا معيقًا في الحياة العملية للفرد فالإحباط الناتج عن التعامل مع المرض الذي لا يتم تشخيصه غالبًا بالشكل الصحيح قد يؤدي إلى تفاقم الحالة.
العلاجات البديلة لمرض الفيبروميالجا
- العلاجات المكملة والبديلة المقترحة لمواجهة الألم والضغط النفسي ليست جديدة بعض هذه العلاجات مثل اليوغا والتأمّل قيد الاستعمال الطبي منذ آلاف السنوات، لكن السنوات الأخيرة شهدت زيادة في الوعي بهذه العلاجات
