- نموذج رقم (40) والذي يحمل اسم “طلب الحصول على صورة وثيقة أو مستند”، كما هو مدون في الصورة التالية.

الأشخاص الذين لهم الحق في استخراج شهادة الميلاد المميكنة، هم “مقدم الطلب، الأب، الأم ، الأخ، العم، الزوجة، الأبناء، الجد، الخال”.
طريقة استخراج شهادة ميلاد كمبيوتر عبر الانترنت:
- قم بالدخول إلى موقع ” قطاع الأحوال المدنية بوزارة الداخلية المصرية“.
- قم بالضغط على خدمة طلب الحصول على قيد ميلاد مميكن.
- بعد ذلك قم بالضغط على خانة وقائع الميلاد والتي تكون في يمين الصفحة وتتيح اختيارين هما ” وثيقة ميلاد أول مرة، استخراج شهادة ميلاد مميكنة مطبوعة سابقًا”.
- ما أن تقوم بإختيار أي من هذين الخيارين يعرض لكم الموقع إمكانية تسجيل الدخول أو إنشاء حساب جديد على الموقع، وهي عملية لا تستغرق وقت كبير من خلال تدوين البيانات الشخصية في خاناتها المحددة وتسجيل رقم الهاتف حتى تستطيع الموقع التأكد من بيانات الشخص الذي يرغب في استخدام خدمات الموقع.
- بعدما تطلب من الموقع شهادة ميلاد يعرض لك خانة لتقوم فيها بتحديد عدد الوثائق التي ترغب فيها كما هو موضح في الصورة التالية.
- بعد الضغط على زر ” التالي” يعرض لك الموقع عدة خانات يطلب منك فيها ملء بياناتك الشخصية، ومنها الرقم القومي وذلك بعد تحديد ما إذا كنت ترغب في استخراج شهادة ميلاد لنفسك أو لشخص آخر.
- بعدما تقوم بملء البيانات المطلوبة تقوم بوضع علامة (صح) للتأكيد على أنك قرأت ووافقت على الشروط المطلوبة، ثم تقوم بالضغط على زر “التالي”.
- بعد ذلك يطلب منك الموقع ملء بيانات مقدم طلب الحصول على شهادة ميلاد مميكنة والذي سيقوم بإستلام شهادة الميلاد، مع تحديد العنوان الذي سيستلم فيه الشهادة والرمز البريدي والمحافظة والمركز التابع له.
- بعد الضغط على زر “التالي” يطلب منك الموقع إدخال الرمز التأكيدي أو رمز التحقق في الخانة المخصصة له لإتمام عملية التسجيل والضغط على زر “إرسال الطلب”.
